To stay an official chapter, the following forms are required:
Official Chapter Agreement (OCA):
Once you have completed the OCA, your chapter is eligible for grant funding and can receive a care package to jump start the year.
Food Tracking Forms: track your pounds recovered at least once per semester
End of Semester Survey: to be completed at the end of every semester

Mandatory Forms for Official Chapters
All FRN Chapters must sign an Official Chapter Agreement (OCA) each academic year in order for your chapter to be active in our network. All chapters must comply with the Bill Emerson Good Samaritan Food Donation Act as outlined in the OCA.

Food Recovery Network financially supports Official Chapters, up to $300 an academic year. Confused how to use the form? Read here for more information. Email us at programs@foodrecoverynetwork.org if you have an idea or event that would require more funding for your chapter and aligns with FRN’s goals. To check your chapter’s funding balance find your chapter under Current Chapters and select the pounds-to-date spreadsheet to view the funding tab. You may have extra dedicated funding, if your chapter has been inactive for 3 years, that money will be re-introduced into the general funding pool for all chapters.
